Job Title: Home Health Physical Therapist

Are you a licensed physical therapist eager to make a difference in the lives of seniors and adults in the comfort of their own homes?

BAYADA Home Health Care is seeking an experienced Physical Therapist to join our team in Raleigh, NC. As a home health physical therapist, you will work with our multidisciplinary healthcare team to provide rehabilitative care and skilled nursing to clients in senior living communities throughout Wake and Durham Counties.

Responsibilities:

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements to assess client needs.
• Develop and implement individualized care plans, including manual therapeutic exercises, gait training, and other interventions.
• Continually assess and revise the care plan, participating with nursing in the multidisciplinary care plan as needed.
• Educate and instruct clients, family members, or representatives in rehabilitative care and activities necessary for independent living.
• Accurately document observations, interventions, and evaluations using a state-of-the-art touch pad tablet.
  Qualifications:
  
  • A current North Carolina Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills, with prior experience with electronic medical records preferred.
